Have you executed an Advance Directive for Health Care? Or has it been over 3 years since you last updated it and all your family contacts have changed or are deceased? Have you registered it so that someone can find it and follow your personal wishes at a time of crisis? Do you have a Vermont specific document?
If you answered "NO" to any of these questions, or if you are wondering about new options, join us to learn more about Advance Directives! Actually doing this process can bring peace for you, your health care providers and your significant others or family members. Bring them along!
Stop by MSAC to access more information and forms so you will be prepared to "do it yourself" or ask more questions in a group session. We will schedule a follow-up session in August to complete the forms and answer additional questions.
Presented by MSAC member Mary Alice Bisbee, M.S., Human Services/Gerontology, Notary Public, and long-time hospital and nursing home social worker.
